Job Description
Relevant Experience: Previous experience in a support worker, care assistant, or similar role, particularly within health and social care settings, is highly desirable.
•Communication and Interpersonal Skills: Strong ability to build positive relationships with individuals and their families, as well as with colleagues and external professionals, fostering an environment of trust and cooperation.

•Organisational Skills: A calm and methodical approach to work, with the ability to manage multiple tasks and priorities, ensuring that care is delivered effectively and efficiently.
Desirable Experience and Skills:
•Experience in working within hospital discharge services, home care settings, or with individuals with complex health and social care needs.
•Familiarity with triage or duty systems and experience in responding to urgent care needs.
